Patient Centered Medical Home

A Patient Centered Medical Home is not a home or building. It is a team approach to providing comprehensive health care in a high quality and cost-effective manner.

As a patient you are the most important person in the healthcare system. Being part of a medical home provides total health care with a team that includes healthcare professionals, family members and/or friends (if you wish) and you.

Primary care and specialty physicians, nurse practitioners and medical assistants will be part of this healthcare team. They will be here to answer your calls and questions, send you reminders about check-ups and tests, and coordinate your care with other providers.

How Our Medical Home Can Work For You

Our patient centered medical home allows you to call out office first. In our office, we strive for personalized care that is safe and scientifically valid, with a focus on prevention to keep you healthy.

We are old style family doctors, backed by teams, a network of specialists, and modern technology.

Our focus is to care for you as an individual in the best way possible. Your physician and your team know you and you know us.

We will address your needs in out office or connect you with other services outside out office. We coordinate your health care with trusted specialists and other healthcare providers. We will help you get appointments and will ensure that your other providers have all the information they need.

We use a safe and secure electronic medical record (EMR) to maintain your information and to communicate with in the network hospitals on lab tests, screenings, and other important information. If you got to an out of network hospital, out office makes all attempts to obtain your information.

The new web-based patient portal will allow you to view your health information and communicate with your team through email.

How Our Medical Home Can Work For You

Learn about caring for yourself, understand your condition and what you can do to stay as healthy as possible. Set obtainable goals and work hard to reach them.

Talk openly with your team, if there are problems, discuss them with your team to find a solution. If the plans or goals are not working, speak up. Feel free to share your good and bad past experiences.

We offer same-day appointments for all or providers. If you need to a physician, please call the office at 718-264-1111 to obtain an appointment.

We accept most medical insurances and if you do not have, we can help you apply for one.

MONDAY – FRIDAY: 9:00 AM – 6:00 PM

SATURDAY: 9:00 AM – 2:00 PM

After Hours care

For urgent questions after office hours, you can all our office at 718-264-1111 and your call will be received by our answering service and a Licensed Registered Nurse/Nurse Practitioner from Night Nurse Triage will return your call within 10-30 minutes depending on the urgency of your situation.

The Nurse/Nurse Practitioner will advise you and if necessary, direct you to the nearest healthcare facility for further care.
