What To bring to your immigration medical exam
- Government issued photo ID, such as passport, state ID, driver’s license, work permit or travel permit
- Immunization or vaccination records
- Copy of your medical history
- Copies of any previous chest x-rays (if any)
- Health insurance card
- Form I-693 (Report of Medical Examination and Vaccination Record)
- Required Fee (we only accept cash)
