How to Prepare for Your Visit
Please bring all your prescription and over-the-counter medications with you at each visit.
WHAT TO BRING ON YOUR APPOINTMENT
- Insurance card
- Photo ID
- List of medications and/or medication bottles
- Medical Records or Discharge Papers (if available)
- Immunization records (if available)
- List of past and present medical providers
- Cash for co-pay (is necessary) (We do not accept credit card payments. Sorry for the inconvenience).
- Completed registration forms and questionnaires
FOR RETURNING PATIENTS
- Please let our staff know if you have had any information changes since your last appointment, i.e. medical insurance, address, contact numbers
- You will be asked to fill out new registration forms annually so we may update your information.

If you are experiencing a medical emergency, please dial 911.
After Office Hours
For urgent questions after office hours, your call will be received by our answering service and a licensed Registered Nurse/Nurse Practitioner from the night nurse triage will return your call within 10 to 30 minutes depending on the urgency of your situation. The Nurse/Nurse Practitioner will advise you and if necessary, direct you to the nearest health care facility for further care.
