The PATIENT PORTAL allows you to have real-time access to your medications, labs, and upcoming appointments. You will also be able to request appointments, request refills, and message your provider. You can also update your information and medical history while on the portal.

This web-based patient portal is to be used for NON-URGENT issues only.

You should have received a letter from our office with an ACTIVATION CODE and instructions on how to create and activate your account so you can start using the PATIENT PORTAL. If you have not received one, please call or visit our office to get your portal activation letter.

For current Hillside Polymedic patients, on-line appointments can be made through the Patient Portal tab.

If you have received your Portal Activation Letter and have activated your account, then you can request your appointment on-line. You can also view all your appointments, past and future, thru the portal.

If you have not received your Portal Activation Letter, then please request for it on your next visit to our office or you can visit us anytime to receive it.
