A Patient Centered Medical Home is not a home or building. It is a team approach to providing comprehensive healthcare in a high quality and cost-effective manner.

As a patient you are the most important person in the healthcare system. Being part of a medical home provides total healthcare with a team that includes healthcare professionals, family members or friends (if you wish) and YOU.

In addition to your medical providers, medical assistants, social workers, dietitians and medical technicians will be part of this healthcare team. They will be there to answer your calls and questions, remind you about check-ups and tests, and coordinate your care with specialists and other providers.
